Hubungan nyeri kepala pada awal masuk rumah sakit dengan mortalitas pasien stroke

Abstract
Background: Stroke is the second leading cause of death worldwide and is one of the leading causes of disability. Headache is often reported by stroke patients, especially in the early phase of stroke. However, the relationship between early headache and mortality in stroke patients is still unclear and requires further research.
Objective: This study aims to determine the correlation between headache in early admission with mortality of stroke patients in Dr. Sardjito General Hospital Yogyakarta.
Methods: This study used a cross-sectional design with secondary data analysis from the stroke registry of Dr. Sardjito Hospital Yogyakarta. The study sample consisted of 551 stroke patients, consisting of 379 ischemic stroke patients and 172 hemorrhagic stroke patients who were treated between January 2020 and December 2022. Data were analyzed using bivariate and multivariate tests to examine the relationship between early admission headache and mortality.
Result: Of the 551 patients, 192 patients (34%) experienced headache at the beginning of admission. Mortality was higher in hemorrhagic stroke patients (33.7%) compared to ischemic stroke (13.2%). Bivariate analysis showed that initial headache on admission was significantly associated with mortality in stroke patients (p <0.05). Patients who experienced headache had a higher risk of mortality than those who did not experience headache.
Conclusion: The result of this study indicate that headache in early admission is independently correlated with mortality stroke patient.

Latar Belakang: Stroke adalah penyebab kematian kedua di dunia dan menjadi salah satu penyebab utama kecacatan. Nyeri kepala sering dilaporkan oleh pasien stroke, terutama pada fase awal stroke. Namun, hubungan antara nyeri kepala awal dengan mortalitas pada pasien stroke masih belum jelas dan memerlukan penelitian lebih lanjut.
Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan nyeri kepala awal admisi dengan mortalitas pasien stroke di RSUP Dr. Sardjito Yogyakarta.
Metode: Penelitian ini menggunakan desain potong lintang dengan analisis data sekunder dari registri stroke RSUP Dr. Sardjito Yogyakarta. Sampel penelitian ini terdiri dari 551 pasien stroke, yang terdiri dari 379 pasien stroke iskemik dan 172 pasien stroke hemoragik yang dirawat antara Januari 2020 hingga Desember 2022. Data dianalisis menggunakan uji bivariat dan multivariat untuk menguji hubungan antara nyeri kepala awal admisi dan mortalitas.
Hasil: Dari 551 pasien, 192 pasien (34%) mengalami nyeri kepala pada awal admisi. Mortalitas lebih tinggi pada pasien stroke hemoragik (33,7%) dibandingkan dengan stroke iskemik (13,2%). Analisis bivariat menunjukkan bahwa nyeri kepala awal admisi berhubungan signifikan dengan mortalitas pada pasien stroke (p <0,05). Pasien yang mengalami nyeri kepala memiliki risiko mortalitas yang lebih tinggi dibandingkan yang tidak mengalami nyeri kepala.
Kesimpulan: Hasil studi ini menunjukkan bahwa nyeri kepala awal admisi berhubungan secara independen terhadap mortalitas pada pasien stroke.
