Abstract
Antibiotik yang irasional berkontribusi terhadap resistensi antimikroba. Tujuan penelitian untuk mengetahui persentase rasionalitas antibiotik pada pasien rawat inap pasien di RSUD dr. Rehatta Jepara menggunakan metode Gyssens serta menganalisis keterkaitan antara rasionalitas antibiotik dengan luaran klinis dan LOS (length of stay). Rancangan penelitian adalah kohort retrospektif dengan data dari catatan rekam medis pasien anak dan penyakit dalam yang mendapatkan terapi antibiotik empiris selama rawat inap periode 1 Januari – 31 Desember 2023. Data pasien anak sejumlah 58 dan penyakit dalam 118. Rasionalitas antibiotik dikategorikan berdasarkan metode Gyssens dan uji statistik Chi-Square atau Fisher Exact test digunakan untuk menganalisis korelasi rasionalitas antibiotik empiris terhadap luaran klinis dan LOS dan multivariat regresi logistik untuk analisis faktor perancu. Hasil penelitian menghasilkan dari 230 regimen antibiotik, 30% antibiotik rasional dan 70% irasional. Kategori irasional paling banyak adalah pemilihan antibiotik yang kurang efektif sebesar 29,1% dan pemberian tanpa indikasi 16,5%. Tidak terdapat hubungan yang signifikan antara rasionalitas antibiotik empiris terhadap luaran klinis (p=0,129,p>0,05) maupun LOS pasien (p= 0,917, p>0,05) dengan faktor usia berpengaruh terhadap LOS.
