Penggunaan gutta-percha measurement cutter untuk mencegah overfilling material obturasi pada perawatan saluran akar gigi insisif sentral rahang atas

Abstract


Obturasi saluran akar bertujuan menciptakan seal yang rapat secara koronal, lateral, dan apikal yang adekuat untuk mencegah infeksi pasca perawatan. Sensasi tug back pada saat trial obturasi sering digunakan sebagai indikator adaptasi apikal gutta-percha, namun prosedur tersebut tidak selalu mencerminkan posisi ideal secara radiografis. Ketidaksesuaian ukuran master cone dapat menyebabkan adaptasi apikal yang tidak optimal dan berujung pada overfilling, yang berpotensi menimbulkan reaksi inflamasi jaringan periapikal. Gutta-percha measurement cutter merupakan alat bantu yang memungkinkan pengendalian panjang dan diameter ujung gutta-percha secara lebih presisi. Laporan kasus ini membahas seorang pria berusia 36 tahun dengan keluhan gigi insisif sentral rahang atas kanan berlubang besar dan riwayat perawatan saluran akar yang tidak tuntas. Pada tahap trial obturasi ditemukan sensasi tug back, namun evaluasi radiografis menunjukkan overextension gutta-percha sebesar 0,5 mm. Penatalaksanaan dilakukan dengan pemotongan ujung gutta-percha sepanjang 0,5 mm menggunakan gutta-percha measurement cutter, berdasarkan nilai taper file F3 sebesar 0,09 mm/mm sehingga diameter ujung gutta-percha berubah dari 0,30 mm menjadi 0,345 mm. Setelah penyesuaian dan obturasi, evaluasi radiografis menunjukkan posisi gutta-percha tepat di ujung apikal tanpa overfilling. Penggunaan gutta-percha measurement cutter berbasis kalibrasi taper terbukti membantu mencegah overfilling dan berpotensi diintegrasikan sebagai langkah preventif dalam prosedur endodontik rutin.
