Crown lengthening disertai retreatment insisivus sentralis kiri maksila dengan restorasi mahkota pasak

Abstract
Keberhasilan perawatan gigi ditentukan oleh perawatan saluran akar dan restorasinya. Keberhasilan perawatan saluran akar ditentukan oleh preparasi saluran akar dan obturasinya. Kegagalan perawatan saluran akar akan menimbulkan keluhan di
masa mendatang dan membutuhkan retreatment. Tepi permukaan fraktur servikal gigi yang tajam dengan ketinggian hampir sama atau di bawah gingival crest dapat menimbulkan gesekan antara gingiva dengan gigi yang berlangsung lama dan terusmenerus
sehingga terjadi proliferasi pembuluh darah dan terbentuk hiperplasi gingiva. Hiperplasi gingiva menghalangi terciptanya seal tumpatan sementara selama retreatment, oleh karena itu dilakukan crown lengthening terlebih dulu pada awal retreatment. Crown lengthening juga akan memberikan tampilan yang lebih estetis dan membantu terciptanya ferrule effect untuk resistensi mahkota pasak. Makalah ini bertujuan untuk melaporkan perawatan crown lengthening disertai retreatment pada insisivus sentralis kiri maksila dengan restorasi mahkota pasak. Pasien mengeluhkan gigi depan kiri atasnya yang sakit selama seminggu. Dua tahun yang lalu gigi tersebut dirawat saluran akarnya kemudian dipasang mahkota jaket tetapi kemudian gigi tersebut patah. Pasien merasa terganggu penampilannya karena gigi tersebut. Perawatan diawali dengan mengeluarkan bahan obturasi dari saluran akar dan pemberian medikasi ekstra oral. Perawatan dilanjutkan dengan prosedur crown lengthening. Kontrol pasca pembedahan 1 minggu setelah operasi menunjukkan luka bekas operasi baik dan tidak ada tanda inflamasi. Luka bekas operasi dipantau selama proses retreatment hingga sebelum dilakukan pembuatan restorasi gigi permanen untuk memastikan hiperplasi gingiva tidak terbentuk kembali. Hasil crown lengthening akan mempengaruhi hasil retreatment dan restorasi. Keberhasilan crown lengthening dipengaruhi oleh prosedur dan tekniknya, ditandai dengan tidak adanya keluhan serta tidak terjadi hiperplasi gingiva kembali
ABSTRACT: Crown Lengthening and Retreatment on the Left Maxillary Central Incisor with Post Crown Restoration. Good dental care is determined by its root canal treatment and restoration. Good root canal treatment is determined by its root canal preparation and obturation. Failure of root canal treatment will cause complaints in the future and require retreatment. Sharp edge surface on cervical fractured teeth with almost the same height or below the gingival crest could cause prolonged and continuous friction between the teeth and the gingiva so that a proliferation of blood vessels occurs and forms gingival hyperplasia. Gingival hyperplasia would disturb the creation of temporary restoration seal during retreatment, therefore it is necessary to have crown lengthening first at the start of retreatment. Crown lengthening will also provide a more aesthetic appearance and help create ferrule effect in post crown resistance This paper will report the crown lengthening treatment with retreatment in the left maxillary central incisor with post crown restoration. The patient complained of left upper front painful tooth. About two years ago the patient received an endodontic treatment and a jacket crown restoration on that tooth but it was fractured and the gingiva became larger. Patient feels disturbed because of the appearance of the teeth. Treatment began by removing material from the root canal obturation and administration of oral medication. Treatment continued with crown lengthening procedure. Post-surgical controls at 1 week after surgery showed good incision and no signs of inflammation. Incision monitored during retreatment process until prior to the manufacture of dental restorations to ensure permanent gingi val hyperplasia is not reformed. The results of crown lengthening would affect the results of retreatment and final restoration. Crown lengthening is influenced by the success of the procedure and the correct technique, characterized by the absence of complaints and permanent disappearance of gingival hyperplasia.
