Penanganan mesial tipping molar II akibat kehilangan molar I dengan L loop

Abstract


Mesial tipping molar kedua akibat premature loss molar pertama merupakan kasus yang sering terjadi. Gerakan mesial tipping molar kedua dapat menyebabkan traumatik oklusi, gangguan fungsional, gangguan prostetik, dan masalah periodontal. Kekuatan ringan dan terus menerus dari spring pembantu diperlukan selama koreksi mesial tipping molar. Koreksi mesial tipping pada teknik Edgewise dapat dilakukan dengan L loop. Laporan kasus ini bertujuan untuk menganalisis efektifitas L loop pada penanganan kasus mesial tipping molar kedua mandibula. Pasien perempuan berusia 27 tahun, mengeluhkan gigi berjejal, gigi tidak berkontak baik dan kesulitan mengunyah di sisi kanan. Pemeriksaan objektif menunjukkan crowding anterior rahang atas dan bawah, malrelasi gigi dan mesial tipping gigi molar. Diagnosa kasus ini adalah maloklusi angle klas II divisi 1 subdivisi tipe dentoskeletal dengan bidental protrusif disertai pergeseran midline rahang bawah; spasing; mesial tipping 16,47; malrelasi serta malposisi gigi individual. Perawatan dilakukan dengan alat cekat teknik Edgewise dengan L loop di mesial gigi 47, 16, dan vertikal loop pada interdental gigi yang malposisi. Hasil perawatan: Setelah 11 bulan perawatan gigi 47 berhasil tegak, malposisi dan malrelasi gigi terkoreksi namun masih terdapat open bite pada gigi 22, 23 terhadap 32, 33. Perawatan pada pasien masih berlangsung hingga saat ini. Penggunaan L loop pada teknik Edgewise efektif untuk penanganan kasus mesial tipping molar kedua mandibula.

ABSTRACT: Uprighting Mesial Tipping of the Second due to loss of Molar with L Loop. Mesial tipping of second molar due to premature loss of first molar is the common case in adult patients. This condition induce the traumatic occlusion, functional disorders, prosthetic disorders and periodontal problems. Light and continuous strength from assisting spring is required during molar mesial tipping correction. Correction of mesial tipping in Edgewise technique can be performed using L loop. The aim of this report is to analyze the effectiveness of using L loops in correcting mandibular second molar mesial tipping. Method: 27-year-old female patient, with a chief complaint of dental crowding, open bite and chewing difficulty on the right side. Objective examinations showed crowding anterior of maxillary and mandibular dentition, malrelation and molar mesial tipping. Diagnosis: angle’s Class II division 1 subdivision type dentoskeletal malocclusion with bidental protrusive, midline shift on the lower jaw; spacing; mesial tipping 16, 47; malrelation and individual malposition. Treatment used a fixed appliance edgewise technique with L loops in mesial teeth 47, 16, and vertical loop at the interdental tooth malposition. The mesial tipping right mandibulary second molar 47 was well uprighted after 11 month of treatment. Malpositions of individual teeth and malrelation were corrected, meanwhile open bite 22, 23 toward 32, 33 has not been corrected and the treatment is still ongoing. Conclusions: The use of L loop on Edgewise technique is very effective for treatment of mandibular second molar mesial tipping.
