Gambaran Efektivitas Triheksifenidil Terhadap Sindrom Ekstrapiramidal Pada Pasien Skizofrenia di Rumah Sakit Jiwa Grhasia Yogyakarta

Abstract


Triheksifenidil adalah antikolinergik yang telah lama dianggap sebagai pengobatan terapi sindrom ekstrapiramidal (EPS). Di Indonesia sebagian besar pasien skizofrenia yang diterapi dengan antipsikotik diberikan obat tambahan triheksifenidil untuk mengatasi EPS maupun sebagai profilaksis. Penelitian ini bertujuan untuk mengetahui gambaran efektivitas terapi triheksifenidil terhadap sindrom ekstrapiramidal pada pasien skizofrenia. Penelitian ini merupakan penelitian deskriptif dengan rancangan cross sectional. Penelitian dilakukan di Rumah Sakit Jiwa Grhasia Yogyakarta. Pengumpulan data dari rekam medik pasien skizofrenia periode Januari-Desember 2024. Kriteria inklusi penelitian adalah pasien skizofrenia berusia dewasa ≥18 tahun yang diterapi triheksifenidil dengan gejala EPS, sedangkan kriteria eksklusi adalah data rekam medik yang tidak lengkap dan pasien dengan penyakit penyerta Parkinson, didapatkan 100 sampel pada penelitian ini. Karakteristik pasien menunjukan lebih banyak usia dewasa (18-59 tahun) 85%, jenis kelamin laki-laki 60%, jenis skizofrenia Undifferentiated Schizophrenia 55%. Jenis antipsikotik mayoritas yang diresepkan adalah atipikal 88% dengan terapi kombinasi 2 antipsikotik 63%, dan gejala EPS yang banyak yaitu parkinsonisme 82%. Hasil penelitian menunjukkan 74 pasien mengalami gejala EPS negatif setelah mendapatkan terapi triheksifenidil. Dosis triheksifenidil yang paling banyak diberikan adalah 4mg/hari, yang diterima oleh 67 pasien dan terdapat gejala EPS negatif pada 48 pasien.
